You can make an appointment by registering online on the main menu of the website www.centrumozimek.pl.
Make an appointmentAfter clicking on “Make an appointment” please select the type of appointment you wish to make:
- tick-borne diseases,
- parasitic diseases,
- pediatric visit
- B-Light procedure.
Then follow further instructions on the screen.
Attention!
- The appointment system is not supported by Safari (iPhone), we recommend using the CHROME browser.
- We do not provide medical consultations by phone or e-mail. All questions regarding prescriptions, exemptions, diagnosis, test results, and follow-ups require taking a medical history and making an appointment.
- Bring the results of all tests ordered by the doctor to your next visit.
- When making a pediatric appointment, please enter the data of the patient (child).
- Please bring your (or patient’s) ID with you to the visit.
- Please come 15 minutes before the visit to fill in the medical documentation (questionnaire).
If you cannot come to your appointment, please cancel your visit as soon as possible via our system.
